Private Camp Contract Team: Camp Date: Date Format mm/dd/yyyy Camp Time: Camp Location Thank you for choosing Spirit Xpress West for your private camp needs. The team should be prepared for 5 hours of intense instruction per day. The outcome of the camp is greatly influenced by the attitude of the team. A team will get the results they want if they are positive, fun, and willing to try new ideas. If there are negative behaviors by the participants, the coach will be addressed. If the negative behavior continues the participant will be asked to leave the session. The environment is another variable that influences the outcome of the camp. Please make sure mats are available, if applicable, good lighting, no distractions and an adequate room temperature. Nutrition is another variable that influences the outcome of the camp. Make sure all participants are responsible for hydration and eating protein and fats for energy. Insurance: Each participant must be properly insured in case of injury. Forms: Each participant and coach must have a signed waiver. The waivers will be collected before the start of the camp day. To register your team, please use the online registration on our website; www.spiritxpresswest.com, submit the head injury form along with the private camp contract to Patty@spiritxpresswest.com. An invoice will be generated after the registration has been received. Payment: A deposit is required to hold the date. A school purchase order, organization check, credit card, or money order is accepted, no personal checks. 25% is due two weeks after booking your date, or your date will be released. The balance will need to be paid two weeks prior to the date or your date will be released. No deposit or tuition refunds.
